Actually, the implementation within PlacementIndex is missing.
Moreover, I think now that PlacementIndex shouldn't implement
QueryResolver directly.

Fix build with gcc 4.3

(Explanation: 4.3 detects when a local typedef redeclares a templated type
from an enclosing scope. This was never legal anyway)

build a separate test-common executable (for everything in src/common)
include a precompiled header for assets.
